Sports Physical Pre-Registration
Amistad Health Clinic will be conducting Sports Physicals at no cost.

If you are under 18 years of age, a guardian or parent must accompany you during the sports physicals.

A MEDICAL HISTORY FORM must be filled out every year. A parent or guardian should fill out the form for the student to join school activities. A sports physical determines if a person can participate in activities safely. It is not a substitute for a general health maintenance exam.

The University Interscholastic League (UIL) only requires sports physicals for students entering 7th grade and 12th grade. If you have questions about your child's sports physical requirements, please contact your school district or athletic department.
